What are the documents you need to prepare for the first screening of the Japan Exchange and Teaching Programme (Batch 2023)?

Arrange your documents in this order.
1. Application Form (one original and two photocopies)
- read the guidelines and the chart sheet of the application form carefully.
- must be typewritten, complete with all the necessary details.
- must be signed with a wet signature (not digital, e-signature).
2. Self-Report of Medical Conditions (one original and two photocopies)
- must be typewritten, complete with all the relevant medical history in the past five years.
- must be signed with a wet signature (not digital, e-signature).
- additional requirement: Statement of Physician (one original and two photocopies)
3. Transcript of Record(s) (one original or certified true copy and two photocopies)
- of ALL universities attended;
- printed digital copies are accepted.
4. Statement of Purpose (one original and two photocopies)
- typed, single-sided, double-spaced
- printed on A4 size paper with 1-inch margin on each side
- maximum of two pages only
- Font Style: Arial; Font Size: 10.5
- Additional instruction (batch 2023): put the number of words at the end of the last sentence of your essay. Type your full name and sign it as well.
5. Certificate of Graduation (one original or ctc and two photocopies)
- certificate of the expected date of graduation (for applicants who have not graduated yet).
- certified true copies of CoG or Diploma and printed digital copies are also accepted.
- two letters of recommendation from your supervisors (if working) and professors (if still studying) with wet signature.
- provide two photocopies for each letter.
7. Teaching or Language Proficiency Qualifications (three photocopies)
NOTE: If you do not have these, it is ok! But, if you do, it will give you additional points!
- PRC License (Teaching qualification)
- TEFL/TESL/TESOL (for English)
- JLPT (Nihongo)
8. PSA Birth Certificate or Passport (three photocopies)
- or any document that shows that you are a Filipino Citizen
9. NBI Clearance (original copy only)
- only applicants with a criminal history are required to submit.
- if you do not have a criminal history, there is no need to include this.
Compile the documents.
One set for the original, another set for photocopy 1, and another for photocopy 2.
You may use paper clips (do not staple! do not put in a clear book! no need to put each set in a separate envelope or folder!)
Attach this form on top of the original set of documents with a paper clip.
